Pharmacokinetics

QuantityFormula
Half-lifet½ = 0.693 / k = 0.693 × Vd / CL
ClearanceCL = k × Vd = Dose × F / AUC
Volume of distributionVd = Dose / C0 (IV bolus)
Loading doseLD = Vd × Ctarget / F
Maintenance dose rateRate = CL × Css / F
Average Css (oral)Css,avg = F × Dose / (CL × τ)
Accumulation factorR = 1 / (1 − e^(−kτ)); = 2 when τ = t½
Absolute bioavailabilityF = (AUCoral/Doseoral) ÷ (AUCiv/Doseiv)
Organ clearanceCL = Q × E (blood flow × extraction ratio)
Time to steady state4–5 half-lives (90% at 3.3 t½)
Michaelis–Menten (phenytoin)Dose = Vmax × Css / (Km + Css)

Patient size and renal function

QuantityFormula
Cockcroft–Gault CrCl(140 − age) × wt / (72 × SCr mg/dL); × 0.85 if female
Ideal body weight (male)50 kg + 2.3 kg per inch over 5 ft
Ideal body weight (female)45.5 kg + 2.3 kg per inch over 5 ft
Adjusted body weightIBW + 0.4 × (TBW − IBW)
BSA (Mosteller)√(height cm × weight kg / 3600)
BSA doseChild dose = (BSA / 1.73) × adult dose
Young's rule(Age / (Age + 12)) × adult dose
Clark's rule(Weight lb / 150) × adult dose
Fried's rule (infants)(Age in months / 150) × adult dose

Dispensing and solutions

QuantityFormula

1 in X1 g in X mL (1 in 1000 = 1 mg/mL)
mmolmg ÷ molecular weight
mEqmmol × valency
mOsm/Lmmol/L × number of particles
Drip rateVolume (mL) × drop factor ÷ time (min)
Isotonicity (E value)NaCl to add = 0.9 g/100 mL − Σ(drug g × E)
Freezing point methodIsotonic = −0.52 °C depression
AlligationParts = difference between the other strength and the target
Displacement valueBase displaced = drug weight ÷ DV

Stability and physical pharmacy

QuantityFormula
First-order t90t90 = 0.105 / k
Zero-order t90t90 = 0.1 × C0 / k
Henderson–Hasselbalch (acid)pH = pKa + log([A⁻]/[HA])
Stokes' lawv = 2r²(ρs − ρl)g / 9η
Noyes–WhitneydC/dt = DA(Cs − C)/h
Arrheniusk = A·e^(−Ea/RT)
Carr's index(Tapped − Bulk) / Tapped × 100
Hausner ratioTapped density / Bulk density

One-liners examiners love

  • Loading dose depends on Vd; maintenance dose depends on clearance.
  • Half-life changes only if Vd or CL changes; dose does not change it (first order).
  • Time to steady state depends only on half-life, not on dose or loading.
  • 1 in 1000 adrenaline = 1 mg/mL (IM anaphylaxis); 1 in 10,000 = 0.1 mg/mL (IV arrest).
  • 0.9% NaCl ≈ 154 mmol/L Na⁺ ≈ 308 mOsm/L.
  • Hausner ratio above 1.25 or Carr's index above 25% means poor flow.

Load the Volume, Maintain the Clearance
LD = Vd × C; maintenance rate = CL × Css.
0.105 for 90%
First-order shelf life t90 = 0.105/k (ln 100/90 = 0.105).
